This form is to be used to check the EMS supply cabinets at the station and notify the EMS Captain of items that need to be reordered.

This document reflects a culmination of evidence-based research, national, state and local standards, which, together provide the latest in pre-hospital care, and reflect the growth of EMS as a specialty in healthcare. While this document, or any document for that matter, cannot specifically address every possible variation of disease or injury, it does provide a foundation for acute care for the majority of patients we respond to.
